Bentracimab is an investigational, humanized monoclonal antibody fragment (Fab) designed as a specific reversal agent for the antiplatelet drug ticagrelor. It functions as a neutralizing antibody that binds with very high affinity to both ticagrelor and its active metabolite in the plasma, forming inactive complexes. This binding rapidly reverses the antiplatelet (P2Y12 receptor inhibitory) effects of ticagrelor, restoring platelet function. It is being developed for use in clinical scenarios where immediate reversal of ticagrelor's effects is required, such as in patients experiencing major bleeding or needing urgent surgery. It has demonstrated rapid and sustained reversal in Phase I and II trials.
