Lanadelumab is a humanized monoclonal antibody that specifically inhibits plasma kallikrein, a key enzyme in the contact system. By blocking kallikrein, it prevents excessive bradykinin production, which is the primary mediator of swelling attacks in hereditary angioedema (HAE). It is approved as a long-term prophylactic treatment to reduce the frequency of HAE attacks.

Function

Plasma kallikrein is a serine protease that circulates in its active form and plays a central role in the contact activation system. Its primary function is to cleave high-molecular-weight kininogen to generate bradykinin, a potent peptide that increases vascular permeability and causes edema. Excessive kallikrein activity drives the pathological swelling characteristic of HAE.
